I got a medical exam in June 2013 and will be posting my application for PR (common law sponsorship) at the end of this month, January 2014. I know that medical exams have to be conducted within the last twelve months. But what if they don't process my application until, say, October 2014? Should I do another medical exam or is it only counted up until the date my application is received? Thanks for your help. I'm sure this question has probably been asked before but I didn't turn up any in my search of the forums.

Why did you do the medical so early? They are valid for 12 months, so it's fine to submit your PR app with the medical info you did in June 2013.
However your medical will almost certainly expire sometime during your PR processing. So when this happens, most likely CIC will send you a request to do a new medical, which will delay your application.
I would be very surprised if you are not required to have another exam, as it is already 7 months old and your application hasn't even reached them yet.
Was the exam that you had back in June conducted by a CIC panel physician, or was the exam for reasons other than your PR application?
If it was just an ordinary physical, you should definitely be prepared to do it again...this time with a CIC approved panel physician. Some Inland applicants wait until they either have AIP, or a request from CIC to have their medical exam. The results are transmitted to CIC from the doctor's office, so they typically have the results within a couple of weeks.
