Vaccination concerns

Hi, i would like to seek help in relation to vaccination records of my kids. We about to depart our place but the problem is until now im still on the process of requesting the immunization records of my children. The public clinic of our place doesnt want to fill the form because the said records loss due to heavy typhoon and flood encountered by our country. What are the things to do? Can we still depart without it having filled out by the physician?
