I did an upfront medical examination this week. When I came home I checked on CIC website about the notification Dr should give me for upfront medical exam. It says that Dr should give me either the first page of the IMM1017B-Upfront or an "Up-front medical notification" print out. However, my Dr didn't give me any of them. The paper they gave me is a eMedical print out. I want to ask you guys if the eMedical print out is sufficient? I called back Dr office and they told me they don't have any upfront forms. I just hope CIC will accept the eMedical print out I am submitting. Please offer whatever you have experienced with regarding this topic. Thank you guys!

I had my medical done in the United States. The doctor that did my exam sent the results directly to CIC. The only thing I left the doctors office with was a receipt that I paid.

if you received an E-Medical receipt with your personal info and picture, then that is what you submit with your applicaiton. i also recommend saving a copy for your own records just in case.
