United States passport waiting time?

Hi folks,
I'm just wondering how long it takes the United States to process an passport?

I have two family members waiting on theirs. My spouse in Texas, filed for his
a year ago...we have been at this for two years. Nothing returns, still waiting.
He received a letter three months ago that it was in process.

Still waiting. What kinds of things could hold up a passport?
