I applied for federal skilled worker program in 2012, and denied on some grounds, I am reapplying for the ongoing one, I need advice on whether to include my UCI number given in the first application or to leave it blank, is it necessary?

Write that UCI number in your current application please.

No point hiding your UCI (If you did,they'd still write same on your file and all your past data will crop up).
So,please include your UCI.
