UCI and Last Entry to Canada : Form IM0008

My husband has been to Canada in 2005 on study visa. But due to family reasons he couldn't study and had to return back in 3 months and didn't come back after that.

Now when i am applying for his spousal visa, in the form (IM0008 Generic application form for Canada and IM5476- Use of Representative) it ask me for his UCI (if known) and last entry to canada .

Sould we mention the UCI from his study permit and his entry to canada in 2005. My consultant said these are mentioned only if the person is still in canada. But i guess UCI will remain same and it should be mentioned in the forms.

Can someone please help me with this ??

Because your husband already was in Canada for some days the information need to be given in the form. Also the UCI number.
