Travel history query

I am from India but I am residing in country A for employment purposes. I travel to India for vacations. So in the travel history section, I was thinking of mentioning the travel dates and reasons as follows:

For example :

01/01/2009-01/02/2009 : Travel from A to India : Visit relatives
02/02/2009-02/02/2010 : Travel from India to A : Employment
03/02/2010-05/03/2010: Travel from A to India : Visit relatives

.
.
.
.

Is this the correct way to fill this form?
