Tourism visa with common-law wife

Hello,

I got 2 weeks of english course in Toronto. I wish to travel to Canada for 4 weeks (18 days in Toronto and 12 days in other cities of Canada) with my common-law wife.

I need get the tourism visa with study permit (SX-1) and my wife need get the simple tourism visa (V-1), but I will be the main applicant.

In which form I need inform the documents of my wife (passport and more)? I don't see on IMM5257 and IMM5707.
