TAX Assessment for PR card renewal

Hi,

I will be putting my application for PR card renewal. I am wondering do I have to submit all the 3 pages of the my TAX assessment (1st Page from CRA says Thank you for TAX assessment etc.) OR Shall I submit ONLY last page where it has actual numbers.

Do Direct printout from CRA website is acceptable (Please Note direct printout do not contain my full SIN number its in XXX-XXX-NNN format where N=Actual Number)?

Urgent help will be appreciated.
