Surname different in transcripts and passport

Recently I applied for a diploma course to the University of Manitoba and it got declined for the following reason

We reviewed your application to the Applied Business Management Program at the University of Manitoba. Unfortunately, your application cannot be considered. It is incomplete for the following reason(s):


1. While you submitted a degree and a transcript, the name on both documents are not matching the name on your passport. If you ever changed your name, please provide a government-issued document evidencing your legal name change.


We would be pleased to re-open your file should the missing document(s) be submitted to us within the next three weeks of receipt of this e-mail. Documents submitted after the due date will not be accepted.

In my college transcripts My name is mentioned as J. Ajay Daniel as J's expansion you can see is Jeyakumar Dickson i.e my father's name but in the passport my surname is stated in full where there's no initial J mentioned. What should I do? Need help.

Get the name in passport changed to the one in degree/transcripts as it's much easier than other way round. Hope your other documents have similar name as your degree/transcripts

But do you think submitting an affidavit form stating that the name mentioned on the passport as well as on the certificates are same. Would this solve the problem?

Depends entirely on the Uni. No harm in trying
