Hello, I am filling out the passport application on behalf of my mother.
My question is that there is a section to fill information to support identity and I have to write document name, number, name on the document etc.
Since I have an affidavit which mentions her name change before and after marriage, I want to ask can an affidavit be used to support identity or it should be an official government-issued document?
Should I just add the affidavit and supporting documents with the passport application?
