Supplementary travel Info - Space insufficient

Hi ALL ,
Regarding filling out the supplementary travels info , do we have to list all the travels made in the last 10 yrs??
I have made several short trips to the states in the last two yrs and the space provided in the form is not sufficient.
What should I do ? Could i attach another white sheet of paper indicating all the travels in it or is there anything else I could do ?

Please Suggest .

Correct, attach a separate sheet. Make sure to mention your name, UCI and DoB on the top,
as well as the name of the sheet you have to attach information to. Then make up a
table, like the one on the original sheet and fill it out.

Second that I have done the exact same thing...
