Supplementary forms information for Pakistan who live in UAE

I am from Pakistan. My wife is canadian national. I live in Dubai and she is in Canada. She wants to apply my visa. She told me fill my side application and then mail to her. But i have some confusion according to guide information i have to fill three addition forms which is

1- Supplementary Information Form - Pakistan
2- Residency questionnaire - Pakistan
3- Authorization to Disclose Information — Pakistan

Is this must to mail above three forms with other forms IMM 0008, IMM 5669, IMM 5406 and IMM 5490? I am the only applicant as a spouse. Please help me

Or these above three forms submits after get file number

How to pack all application? is in single envelope? how to arrangement of all forms and documents? Are all forms will only paper clips only?
