I am sponsoring my wife in costa rica, i ran into this document right now : supplement to schedule 1 does this form need to be filled out by her and signed? it's weird, when I printed all these documents I had not ran into this for some reason, I guess they updated and added some other forms to it, they also added IMM 5604 to it, I guess that has to be filled out as well? thanks.

Yes, she needs to fill up that paper, supplement to Schedule 1, and attach it to the Schedule 1 form in her application.

The other form, did you mean IMM5406? Additional Family Information? Yes, it is necessary that she fills that up too.
