I have received the medical request form now, do I attach it to her application online or do I contact their office to let them know.

Yes they have sent the medical to them already before she got the request for the medical tests. The hospital said I would have to send the form and the results to immigration. I got confused there.

This doesn't make sense. You will have to confirm with the physician if they already sent the results, if so then that's it, if not then you will have to send them.

Hi All, I have the same question. I have got the medical test done for my parents after I submitted the application but before I get the medical test request.
When I ask the hospital/clinic to send the medical report to IRCC based on UCI number, they are asking for UMI or IME id.
