Hi,

I am a Canadian PR and would like to apply for SuperVisa for my mother. The Date of Birth that I had mentioned in the Family Information on my PR and student visa applications previously are different from the one on my mothers passport as she was born in a village in India and lacking any formal education she didn't had any educational or other documents mentioning her Date of Birth. Someone under similar situation suggested to add an Affidavit by my mother stating her Date of Birth as on her passport and submitting with a notarized declaration from me. I will greatly appreciate any advice or suggestion.

You can email CPC-Ottawa along with a scanned copy of your mom's passport to inform them that you had given the wrong date of birth for your PR application. This will suffice. Affidavits and notarized declarations are not required
