Super visa extension - currently in Canada

My in-laws are currently here in Canada under Super Visa. They arrived on Feb 2015 and will be reaching their 2nd year on Feb 2017. Their visas are valid until May 2019. Here are our questions:
1. Can we extend their stay without leaving the country? If so, can we apply for another 2 years extension?
2. If not, are they required to exit and come back from country of origin (Philippines) or can they re-enter through USA?
3. Are they required to get the $100,000 medical coverage for a year before applying for the extension or before re-entry?
