To be sure I would ask them for another letter quoting your surname and firstname (I assume that's what you mean by middle name) only. This is only my opinion though. It should generally look the same as your passport.

What do you mean "Father's name/Middle Name" on passport? There's no middle name field on an Indian passport
I'm assuming you're Indian - an Indian passport has two name fields
Surname -
Given Name -
Does your passport say
Surname - Shah
Given Name - Raj
or does it say
Surname - Shah
Given Name - Raj Kiran
?
If it's the former (Raj Shah), your offer letter should say "Raj Shah". If it's the latter "Raj Kiran Shah", your offer letter should say "Raj Kiran Shah"

But on offer letter as university asked for middle name / fathers name during application so according to that
