SPP VISA - Medical Form

Hi, I have done my medical test a month ago and I am going to apply for VISA next week. I got only the tracking sheet form diagnostic center. But the SPP checklist requires below info.

Copy of medical examination form IMM1017 if you opted to undergo a medical examination before submitting your application.

Please help, where will get the IMM1017 form? I also got the below info from CIC website/

The doctor will sign the form and mail the first copy to the Canadian Regional
Medical Office (RMO) with the results of the examination. You will be given the
second copy as proof you underwent the medical examination. You must
include this copy with your immigrant application when you send it to your
sponsor. Photocopies will not be accepted. If you lose it, you will not be able to
obtain a duplicate;you will have to redo the medical examination and pay any
associated fees again.

The copy which the doctor gives you is the IMM1017..

Hi, thanks for your reply. I got only the tracking sheet from doctor not form IMM1017? Should I approach the doctor again? Please help me.

Does that sheet has your pic, passport details and e-medical written on it..? If yes.. Then that is it.. Your IMM1017
