1: There is no urgency for inland because the spouses are already living together, for outland the emphasis is on reunification - bringing spouses back together who have been apart
2: It is my understanding that it's counted from the date of receipt
3: By email if you provide an email address, by mail if you do not

As to your 4th question, there's no guarantee but you should have an acknowledgement of receipt by 3 months and your ecas will tell you the date that it started processing. In my case, the initial date of processing was the same day as I received AOR1, almost exactly 2 months after submissions date.
