I am a Canadian citizen who got married in Toronto August 2011, my husband is a US citizen living in New York, I want to start the application process so he can live in Toronto permanently. What forms do I choose? Does it matter where I live or my spouse? and do I send the forms to Ontario office or New York since the Detroit office is closed.

If he is continuing to live in the US, and you in Canada, you are applying Outland.
You send all forms and proof etc to Mississauga, and from there they will probably be sent to Ottawa for processing. You just have to select where you want to go for an I terrier if they choose to interview you.

Hi Kiwhi - it doesn't matter where you live (but if you are living outside of Canada you will need to prove your intent to move back when your spouse is approved for PR), and your spouse can be living in another or "visiting" with you in Canada while the application is processing. If you choose Outland processing all forms go to Mississauga (and then will be forwarded to Ottawa - yes, Ottawa now processes almost all US applications...they forward a few to LA or NY, but you don't decided..Mississauga does) and if you choose Inland they go to Vegreville.
