I am Canadian and just married an American in Oct 2018. I want him to live here. We are going to begin the application process in Jan 2019. He has CKD and on peritoneal dialysis. He can't wait 3 months for medical care. Any advice? Can he come here and just go back to the US for medical appointments until he gets OHIP?

That will work provided he lives in Canada for those three months and just takes day trips to the US.
 
Will he have to report in the US that he is living in Canada?

No idea. That's really a US medical coverage question - not an immigration question. I guess he'll need to look at the policies around his coverage.
 
If he is receiving state coverage like medicaid he has to live in the state. How is he going to commute to dialysis?
 
Has his family been tested as kidney donors? If they do he should remain in the US because a US citizen will not be able to donate a kidney in Canada unless they pay for their own surgery and care as a donor.
