sponsored spouse/partner questionnaire

I was checking my documents in preparation for (finally!) sending off our application forms, when I saw on the document checklist: 'Spouse/Partner Questionnaire'. What!?!?!

When I downloaded the forms, there was no such thing, and the instruction guide doesn't mention it at all. Is this some kind of typo for an outdated form? I did manage to find the form by googling it directly, and it's 'IMM 5490 (04-2008) E'.

Do I need to fill this out? And what's the deal with it being mentioned in the document checklist, but nowhere else?

If you're sponsoring your spouse under a family class category, this is where you need to go to get all the forms.
cic.gc.ca/english/information/applications/fc.asp

You will fill the first part and your spouse will have to fill the other two parts and if you click on part2 of the package you will see all the forms there including the questionair...good luck!!!

Forgot to mention that the link is for applications that will be processed outside Canada.

Thanks. Ahhh, I see my mistake. I clicked the link for 'Family Class' instead of 'Spouse, common-law or conjugal partner and dependent children'. Whoops!
