The sponsorship application seems to be geared more towards immigrants getting their family members here I am Canadian.
Is there a different application form that would be less confusing?
question #9 asks if I am a permanent resident or a naturalized Canadian?
Then goes on to ask what Canadian visa office issued it.
what do I answer to these questions? N/A??? or do I leave them blank?

Just put N/A since the instructions say not to leave anything blank.

For 9 put `no`: you are neither a permanent resident nor a naturalized Canadian.
