Yes. Probably better to have it typed as opposed to handwritten though.

Period of employment is the date from which you started. Perhaps just show your employer the question from the form and ask them to write something.

They could simply do something like

To whom it may concern, (or Dear Canadian Immigration, whatever you want)

My name is Jack T and I am a manager at XXXXX company. I am writing to you today on behalf of John D. John has been working for our company since January 2016 as a full time employee. He works 38 hours per work at a rate of $15.50 per hour. We expect John to continue to work for us long term etc
