I had to take a break of immigration stuff. I see application forms in my dreams now.

I'm confused by these 2 things in our application guide. The first one "Non-accompanying family members Declaration", this only refers to children and spouses right? Surely they don't expect the applicants parents and siblings to go through this?

Second thing is the "Identity and Civil Status Documents"... We will provide evidence of our common law relationship. A birth certificate is also required from the applicant?

We are using the guide for Latin America, if that helps.

Correct, the non accompanying portion is in regards to dependent children/non dependent children. Parents and siblings are listed on form 5406 and are not required to do medical or background checks.

Yes, your spouse is going to need his/her birth certificate (translated if necessary to either English or French) and also needs a passport.
