Has anyone immigrated to Canada with a child after a divorce? My son's husband and I are on good terms, and I have custody, but immigration would like a signed affidavit from him saying that he is allowing me to immigrate with our son. No problem, he said he would do this for us.
My question is, there doesn't seem to be a form and I don't want to get it wrong since it's time sensitive and he has to mail it from the states, has anyone else done this? Do you have any thoughts what I should make sure is in the letter?
I have included his name and contact information and then just, simply, "I (his name) am the father of (son's name) and give my consent to allow him to immigrate with his mother, (my name)."
And then, of course, the part for the notary public to fill out. Is this enough?

You need him to fill out, sign and notarize an IMM 5604 form.
You can search it on their site with other sponsorship forms.
You can either print and get it to him, or if he can, download and print it and deal with it.
It has to be their form, not just a note, as we tried that, but not good enough. In my case, the teen daughter is coming from the US to Canada with her mother, my wife.

OMG thank you SO MUCH!!! ;D This is very helpful. I googled for some sort of form but came up empty and their letter said nothing about a form. Thankfully I was able to send the form to my ex-husband before he did anything with the letter I wrote.
Just make sure he reads it 2-3 times, just so he doesn't forget anything, use full name of child etc, have a notary sign and stamp it and you should be fine.Have it sent to you, then forward it on.
