Hi,

I have a child, and I cannot provide the medicals of her as I have no contact with my ex-wife, and I am sure she would not allow her to provide any. I have attached an affidavit stating that I have no contact with my wife and my child is with her (the affidavit is just to indicate that custody is with my wife, and we decided it amicably), but I didn't explicitly state that I wouldn't be able to provide any medical records. Should I attach a letter of explanation stating the same upfront, or should I wait for the request for medical request and explain then?
