I filled in the link of the form provided and typed my name as the signature (which the form asks for) and uploaded the document via my profile. But it also states bearing an original signature. Should I raise a web form, again with a pen signature, scan the document and send it in?
Thanks for your prompt reply. I uploaded the document with my full name written the same day I received the ADR. I am planning to upload the new document by raising a web form on the IRCC website. Will that be an issue? Also, the web form asks me for a Visa processing office, which I am unaware of, any tips/ suggestions on that?
Send the signed doc via webmail. If they are not satisfied, they ask you to send it through your account. Just mention the visa office that processes for your country.

I recently submitted as well. But they had provided me the link and that was the eimm5669 version. Also, at the time of uploading it to the placeholder, it was only accepting the validated and digitally signed pdf and not any other pdf that I included, i.e. with the scanned and physically signed last page. The eimm5669 version was better in the sense that you could add additional rows for information, unlike the other one, for which you need to add an additional sheet manually. Content wise they were basically having the same questions, so if they have not specified which one exactly, you can try the eimm5669 version because I was literally asked last week to submit that.
