If an application for work permit has been refused (LMO not approved) - do I need to answer YES to any of the questions
-previously sought ...or applied for Canadian immigrant or perm resident visa or visitor or temp resident visa?

If the actual work permit was submitted and refused then you would answer yes to them both. You applied for a temporary resident permit and it was refused. You can give a detailed explanation in the box provided under the list of questions
QC I beleive so. Our consultant said yes and listed below the 2 work permits that my bf had been issued. As well as the work permit refusal. Although many people haven't listed them and nothing was never said.
QC considering you're currently in Canada on a work permit and would have had to send a copy anyhow I wouldn't worry about it too much
Thanks for your reply Kelkel. I was wondering (stressing, really) if I should write an explanation and send it to the visa office. I was thinking of mailing one other thing anyways and could include the two things together. Of course, I did include copies of work permits and explanations of everything in our relationship history, but for some reason, it did not even occur to me that a work permit was also in included in this question.... Sigh.
Don't stress about it. It's a common mistake and while it's always better to answer correctly the first time, your client ID links every application, permit, visa, etc. you've ever had with CIC through FOSS and CAIPS. They can see what you've done with them no matter what you answer.
The only time you need to worry is if you are trying to hide something from CIC by not answering these questions appropriately. So if your intent was bad, your results will be bad.
In most cases, this error is pretty innocent and gets corrected within your application by including your work permits, visas etc. or in your narrative where you may say "I applied to visit my spouse in Canada and my visitor visa application was refused", etc.
If everyone was refused for a simple goof up, probably more than 50% of all applications would be refused!
