Sample letter verifying relationship with maternal uncle

Hello,

I have received an ITA to apply for my permanent residency. I need to provide documents which prove my relationship with my maternal uncle. He does not have a birth certificate and his passport does not have his parents' name.
So my uncle has to write a letter stating his relationship with me and get it notarized.
I was wondering if anyone can help me out with a sample/template for such a letter. Any additional pointers will be very helpful too.

1. Won't help you at all. I could write a letter stating that you are my nephew. Birth certificates, school leaving certificates, showing the same parent(s) as either you mother or father.

Agree with above that a notarized letter is pointless. Your uncle needs to get the proper documentation.
