It seems Tokyo office is not that fast as I thought... your time line shows it took you nearly 6 months to get the med req nearly 6 months and PPR after 2 months of your med done... I am with the same NOC as you but in different visa office.... still hanging in process after nearly 5 months since AOR..

My passport came in Registered mail. The mailman asked me to put a stamp (hanko) or signature before handing a big envelope to me.
In the big envelope you'll get your passports, Confirmation of Permanent Residence and a letter with instructions. In my case, they also returned a couple of original documents that they didn't use or weren't necessary.

We are performed the medical on 13.04.2010 and I asked the embassy about time line but they not answered.

Try sending a case specific inquiry to your visa office and ask for status update. I honestly don't know why but it does take some time and it is normal. I believe patience is the key. In my case, I sent a few case specific inquiries and I was able to get a reply very soon after.
