I went to the US by land in March, but I forgot to return the I-94 slip when I re-entered Canada, specifically Vancouver. The form is still valid until mid-September 2017, but I plan to fly to the US in early September 2017 (a few days before the expiry date). Do I need to send the form + supporting documents to the address stated in the US Customs and Border Protection's website? Or can I just bring it with me when I re-enter the US?

Just bring it with you when you re-enter the US.
(You clearly left the US before expiry + it's not yet expired.)
You'll likely get a new I-94 (electronic). I was in a similar situation as yours, though I had one month left on my I-94 for a one-week stay, they issued me a new one with another 6 months anyway.
