We are in the process of applying for a TRV for my wife....should she show a return ticket first or is the TRV necessary to get the ticket?

I don't think they are linked at all. If you get refused for the TRV, the ticket will possibly be wasted.
You don't need a visa to BUY a ticket, only to use it.
We applied for TRV for my wife at the VACS in Bangkok nine days ago and expect a response (hopefully positive!) in about a week. My wife also has a PR application pending. Is it possible to extend a TRV in case the PR has not yet been finalized?

Yes, it's possible to extend the visitor status.
That's up to the issuing visa officer.
