Click on Miscellaneous
Look under certificates section. The requirements differ depending on whether you got your current passport from the Consulate in SF, or elsewhere. Click on the situation that pertains to you, there's your info.
If someone else has experience with this Consulate that suggests this info is not correct, that's another matter...
i am actually looking at the embassy website under passport issued other than SFO but their is a little confusion. on guidance for mailing instructions page it says send original passport
GUIDANCE ON MISCELLANEOUS SERVICES: Supporting Document
Irrespective of the nature of service, the following minimum of documents should invariably be submitted for all passport, miscellaneous and PIO card services:
* All previous passports in original, current passport in original and photocopy of the first five, last two pages, and of the page containing visa of the current passport. Please remember that Miscellaneous Service can be rendered only against a valid passport
* Proof of Status:
o Clear colored photocopy of Green Card, Employment Authorization Document or other visa documents
o If application for Green Card/EAD/Visa is pending/being processed, document should be submitted
* Proof of Residence:
o Driving license
o PG&E, Water or landline telephone bill displaying applicant’s address
o House Lease Agreement
o Bank/credit card/mobile phone statements are not accepted as residence proof
and on the "For holders of passport issued by any other Passport Office/Indian Mission/Post" page it says
Note:
As it takes around 30 business days to issue PCC, you must submit photocopy of passport at the time of applying. As soon as the clearance is received from the Passport Issuing Authority concerned, you will be called (provided you have mentioned your telephone number in the application) to submit your original passport for stamping of PCC.
Agreed, it's confusing. I think the "Note" overrides the other instructions regarding the original passport. Submit only a photocopy of the passport with the application, they will call you later for the original passport. I think you still need to submit the Proof of Status and Proof of Residence documents.
