Replacement of Citizenship Certificate!!!!!!!!

Hello all,

I need advise. I got my legal name changed and now i have to apply for new replacement citizenship certificate. Please advise what sections i have to fill in the application.

My point was that you can still use the original certificate with the name change certificate.. I am not sure of you need to get a new one . My understanding is that you don't need it unless you want the first passport .for renewal you can still use the one with the previous name and the name change documents .
