My company and my boss is not agreeing to give me the reference letter.
But my colleague is ready. Do I need to mention the relationship with the colleague.

What should i write for the colleague
The letter should be in the format of from 'colleague' to 'whoever it may concern'.
Remember that this is a workaround so you will need to get this notarized before submission. You will also have to include a detailed LOE highlighting your inability to get a more 'straightforward' reference letter from your HR/supervisor and attach all the supporting evidence of your employment and finally request the officer to consider your application as complete and process it on its merits.

The company has been acquired by another company. I think i will have to explain that in LOE.
