Regd Additional Family Info IMM5406

Today I recieved an email asking for "REQUEST FOR DOCUMENTS AND PASSPORT COPIES" for my baby whom I am sponsoring, in the email CIC have asked to fill in the form Additional Family Info IMM5406 -

This form has to be filled for my Baby(10 months) who is the Principal Applicant, who is sponsored by me, my question is who will sign on this form should I sign on his behalf or should I leave blank just writing his name in place of signature. Please help...

This link says we dont need to send this form for Applicants less than 18 years, but I received this request from CIC to submit the form for my baby.

Bummer - maybe its a mistake. Fill it out, sign and write a letter suggesting that he is 10 months old
