Regarding ECA for degree obtained from Indian university

Hi seniors,

I have done engineering from one of the Indian university and I am in process to get ECA done.
My university's Registrar office has attested all the mark sheets and graduation certificate and provided me sealed university envelope.

Question: However, they are not signing Academedic Record Request form (http://www.wes.org/ca/required/International_Transcript_Request.pdf) provided by WES which facilitates the attestation process from university. Is that signed form really required? If I submit all the documents to WES without that form will that be Okay?
