i am applying for my own so am i fill the use of representative form or
can i left with blank page......

If you u not using any representative u do not need to fill any representative form. and follow the forum http://www.canadavisa.com/canada-immigration-discussion-board/applications-after-266-in-any-noc-connect-here-to-get-the-status-t49421.0.html and u will get all the guidance

Leave it blank or write. "n/a". I think if u doing ur own then there is no need for that form.forget abt that. Go thru the website properly everything is there.
 
no need to send that form just don't tick the form in covering sheet.
