Raising CSE for Address history - whether looks ok ?

I'm providing a LoE for address history on my own as I have provided only permanent residential addresses in the address history.

What I missed providing is temporary addresses : my hostel addresses, address of places where I stayed while doing internship and places I stayed while working . All of them are in one country but different states.

What should be the format for my LoE:

1. CIC type: Home then Hostel then Internship place then Hostel then Home then Hostel & so on without gaps.

This i've mentioned for ease, I'll be providing all in tabular format with dates & all

2. Simple Format: only telling temporary addresses that I've missed with dates

Also another doubt floating in my mind is , if all these temporary addresses should reflect on my PCC ?? My PCC has only my permanent address on it.

If you are showing experience of working somewhere else then your residential address ,then you need to provide the address of work place .
