I am an Indian citizen living in US for the past 5 years.
I have not traveled to any other country outside my country of origin (INDIA) or country of residence (USA) in the last 10 years.

Take out your passport. And see entry ------ exit stamps on the visa pages.
Mention them all > from recent to old [for last 10 years]

I was doing my MBA in UK and during the period I made two trips back home. Do I need to mention those as well .
I also visited Moscow on a transit visa for 10 hours . Does it need a mention too ?

Dear Ankul,
You only need to mention any visit outside your home country and that usually goes with entry exit stamps on your visa [you can get dates from there]
If during transit, you went outside the airport with the entry stamp of that country then you need to mention. If you remain within the airport then you do not require to mention it.
