Question on Appendix B in COPR

Hello all,

We have received a request from CIC to submit our passports along with Appendix B containing the updated passport information.

Question regarding Appendix B - It is a form embedded in the email. Is it acceptable to print out my email and fill the Appendix B form to be sent to the CIC? I searched online and no luck finding an Appendix B form in the PDF or Word format.
