Question around PCC from Indian consulate in San Francisco

I emailed the embassy, and they said to put Indian address in 8(a) where someone can be there to verify you.

Is it 27 or 28$ ?

$27 . Indian Welfare fee has reduced to $2 from $3 starting September 2017.Message on SFO consulate site:

"ICWF charges for miscellaneous services would be $2.00 per application instead of $3.00 w.e.f. September 01, 2017"

So when you stick picture on miscellaneous form and personal information form do we sign across it ?

Cross sign only where it is mentioned, i.e. Personal Particulars Form.
Misc application services only require you to paste your photo.Sig not required.

Okay.I somehow feel, one should put current residential address(in my case that is USA).

I agree thats what the question looks like but i followed the tried and tested process.
Also does any one know if one should apply for PCC again if he/she visits India after PCC has been received?
