Question about Work Permit when I apply for sponsorship

I have a question.

I am doing with my common-law sponsorship the open work permit application to send both at the same time. Also at this moment I am working with a Working Holiday Permit. My question is, what of this boxes should I mark in the open work permit application?

These are the options that I have:

-A work permit with the same employer
-An initial work permit or a work permit with a new employer
-Restoration of your status as a worker
-Temporary resident permit

Just make sure the OWP is different (like a black heavy paper clip). They will sort it out.
