Question about Schedule A request

Dear All,

I have received a request to fill out a Schedule A form and IMM5562 Travel Information. Letter was directed to me (principal applicant) and has my UCI number.

I am confused whether they need a Schedule A for my spouse as well, since at the top of the form it says that the principal applicant and spouse should complete their own copies.

Searched the forum but could not find any answers to this, hope somebody can enlighten me about this.

I filled for both myself (principal applicant) and dependent (spouse). there is no harm in filling Schedule A for both

I received one with my Spouse's name only. I only sent his. I have seen another persons letter when they needed both, both persons were quoted on the letter.
