Question about GCMS notes.

Hello, I am applying for GCMS notes for both myself and spouse. I have hired a lawyer to do the application on my behalf and is the authorized representative for both of us. While filling the consent form, it said “designated representative can apply for information”. When I called the they were not able to provide information regarding spouse application although I am primary applicant in the application. Now, who and how to apply for GCMS on both of us?
