Question about Form 5476

I am in a dilemma. I am using a lawyer who says it is ok to fill out only 1 FORM 5476. However, I see in the cic.gc.ca website the Form is mentioned under both Sponsorship Forms and under Immigration Forms.

Now, is he right or we should fill out only 1 Form and I will sign as the Applicant (in the whole application my wife who I am sponsoring is shown as the Applicant naturally) and my wife as the Sponsor?

If we need to fill out 2 form 5476 then how is the signatures be arranged in each Form (i.e. who sign as teh applicant in each of the Form)?

On the Form 5476 - Item10. Section D – Your Declaration

Your spouse must sign in the box provided down to your box.

Send the form for the signature of spouse along with other papers.
