I need to apply for a Canadian passport for our 9-month-old baby, and I have a few questions about filling out the form:
1. Does the “given name” include the “middle name”?
2. "For the full name, is the format 'first name + given name + last name'?
3. For the “registration number” required in the “birth certificate” section, should we use the long form’s or the short form’s registration number?
4. For the birth certificate, should we provide the long form or the short form?
