In the part of "Date Obtained/Effective Date(YYYY/MM)", should I write my birth date (1986-08-30) or only the birth year and month (1986-08)? From the wording, it requires a "date", but in the bracket, it indicates it should be YYYY/MM,which only asks for year and month.

What should I write?

Just put Year & Month. Even if you click on the little dropbox calendar to choose an exact day it only actually prints-out the year & month anyway.
